Short answer: Global developmental delay (GDD) means a child under 5 is significantly behind in two or more areas of development, such as movement, speech and language, thinking and learning, social skills, or daily living skills. It affects an estimated 1 to 3 percent of young children. GDD describes where a child is now; it is not a prediction of where they will end up. The usual next steps are hearing and vision tests, a doctor’s evaluation to look for a cause, and early therapy in every area that is delayed. Therapy does not need to wait for test results.

Key takeaways

  • GDD is diagnosed in children under 5 with significant delays in at least two areas of development.
  • According to the Canadian Paediatric Society, GDD affects an estimated 1 to 3 percent of children under 5 worldwide.
  • Hearing and vision should always be checked, even if the newborn hearing test was normal.
  • A doctor may recommend genetic testing. A cause is found in many children, but not all.
  • Start therapy straight away. Early intervention across speech, motor, play and learning gives the best results.

What “global” developmental delay means

“Global” means more than one area is affected. A child who is only late to talk has a speech delay. A child who is late to sit, walk, talk and play has a global delay. The areas looked at, and the therapies that help each one, are:

Area of developmentWhat a delay can look likeWhat helps
Gross motorLate sitting, crawling or walking; poor balanceOccupational therapy, and physiotherapy where needed
Fine motorDifficulty picking up small objects, holding a spoon or crayonOccupational therapy: Jaipur · Sikar
Speech and languageFew words, not understanding simple instructions, limited gesturesSpeech therapy: Jaipur · Sikar
Thinking and learningSlow to learn new play skills, shapes, colours or routinesSpecial education: Jaipur · Sikar
Social and emotionalLimited interest in people or shared playPlay-based therapy and parent coaching; ABA where needed
Daily livingNot starting to feed, dress or toilet themselves at the expected agesOccupational therapy and parent coaching

What causes global developmental delay?

There are many possible causes, and in some children no single cause is found. Common ones include:

  • Premature birth or low birth weight
  • Difficulties around birth, such as a lack of oxygen
  • Genetic conditions, for example Down syndrome or Fragile X syndrome
  • Infections during pregnancy or in early infancy
  • Differences in brain development
  • Thyroid problems, especially if not picked up by newborn screening
  • Severe illness, poor nutrition or very limited stimulation in the early years

Tests your child’s doctor may recommend

GDD is diagnosed and investigated by a paediatrician, developmental paediatrician or paediatric neurologist. Following guidance such as the Canadian Paediatric Society’s position statement, the first steps usually include:

  • A hearing test, even if your baby passed the newborn hearing screen
  • A vision check
  • Genetic testing: chromosomal microarray is recommended as a first-line test when the cause is not clear
  • Other tests depending on your child’s history and examination, such as thyroid function, metabolic tests or a brain scan

Finding a cause can help with medical care, planning and family decisions. But therapy works on your child’s skills either way, so start it while tests are under way.

GDD, autism and intellectual disability

The term GDD is used for children under 5 because standard intelligence tests are not reliable at young ages. As children grow, some catch up substantially, some are later found to have an intellectual disability, and some are diagnosed with autism, which can occur alongside GDD. Regular reviews track how your child is developing and update the plan.

What helps a child with GDD

  • Early, regular therapy in the areas that are delayed, ideally coordinated by one team with one plan
  • Parent coaching, so everyday routines like meals, bath time and play become practice
  • Special education and school readiness as your child approaches school age
  • Regular reviews of progress against written goals
  • Medical follow-up for any underlying condition

Everyday ways to help at home

  • lots of floor time: let your child move, roll, crawl and climb rather than spending long hours in a walker, pram or lap
  • talk about what you are doing, and wait for your child to respond with a sound, gesture or word
  • offer simple choices: “Banana or biscuit?”
  • let your child do part of every self-care task, such as pulling off socks or holding the cup
  • keep a predictable daily routine
  • limit screen time, which takes time away from talk and play

Questions to ask your child’s doctor

  • Which areas of development are delayed, and by how much?
  • Has my child’s hearing and vision been checked?
  • Do you recommend genetic or other tests, and what would they tell us?
  • Which therapies should we start now?
  • When will you review my child again?

Getting help in Jaipur and Sikar

Paramount Child Development Centre offers occupational therapy for children at both of its centres, with the same assessment approach and the same team model: our Durgapura centre in Jaipur, and our Silver Jubilee Road centre in Sikar, which also serves families from Jhunjhunu, Neem Ka Thana and the wider Shekhawati region. Choose the centre closer to home; regular therapy is much easier to keep up when the journey is short.

Frequently asked questions

Will my child catch up?

It varies a lot. Some children with GDD catch up substantially, especially when the cause is treatable or the delay is mild. Others continue to need support. Early, consistent therapy gives every child the best chance of progress, whatever the cause.

Is global developmental delay permanent?

GDD is a description of current development in a child under 5, not a permanent label. Children are usually reassessed as they get older, when a more specific diagnosis may or may not be made.

Is GDD the same as autism?

No. GDD means delays across several areas of development. Autism involves differences in social communication and behaviour. Some children have both, which is why a full developmental assessment is important.

Should we wait for test results before starting therapy?

No. Therapy works on your child’s skills regardless of the cause. Starting early, while tests are under way, avoids losing months of valuable development time.

Which doctor diagnoses global developmental delay?

A paediatrician, developmental paediatrician or paediatric neurologist diagnoses GDD and arranges any medical tests. Therapists at a child development centre assess each area of development and provide therapy.
